Responsible for obtaining releases from physicians, schools and courts. Maintains tracking system and enters client information into database. Must audit charts on a weekly basis to ensure all information is current and complete. QUALIFICATIONS: Must possess a Bachelor's degree with 1-2 yrs. Facilitates academic and life-skills development and leads groups to discuss pertinent issues relative to at-risk youth. Monitors youth as they transition to each segment of the daily schedule. QUALIFICATIONS: Bachelor’s degree in social work, education, sociology, psychology, humanities or related field; minimum of one year’s experience working with children in school systems, community centers, church groups or after-school programs; ability to work within and relate to an intercultural and multiracial work environment. Current Pennsylvania driver’s license and the ability to drive a fifteen –passenger van.
